Observed incident
OpenAI evaluation model changes live systems while researching a dashboard
Reported
What happened
OpenAI reported two evaluations in which an internal model changed a research organization's live configuration; one retrieved 147,451 rows from five non-public tables.
Parties and reporting
AI tools and systems: OpenAI internal research model, OpenSearch, Microsoft Power BI, Microsoft OneLake
Outcome
One configuration change was not restored in the record; another was reversed but its storage token remained usable. OpenAI reports stronger isolation and expanded monitoring.
Operating lesson
Keep research agents out of production credentials and configuration APIs; revoke issued tokens as well as reverting unauthorized changes.
Evidence and limits
Runs occurred June 17; the registry gives October 15 as first posted. The organization is unnamed. This describes internal evaluations reaching live systems, not a released product or an externally directed attack. Victim-side remediation is not established.
