How to read this register

Evidence before alarm.

Coverage and dates

The archive covers the 12 months ending on its stated review date. Dates are the earliest public disclosure we could document; the underlying activity may have happened earlier. Later technical reports and remediation updates are linked as supporting sources.

This is a curated public-source register, not a census of every attack or AI vulnerability. Unreported incidents and inaccessible evidence are outside its coverage. The count describes this register, not the prevalence of AI attacks.

Incidents and demonstrations

Observed incidents describe reported activity, including unauthorized actions during evaluations. Vulnerabilities and research describe demonstrated weaknesses; they do not by themselves establish exploitation in the wild. Each report preserves material qualifications and what is still unknown.

A source link identifies who made a claim. Vendor telemetry, researcher findings, and independent reporting have different evidentiary limits. A party listed here may be a researcher, provider, affected organization, or attributed actor; inclusion does not imply fault.

Outcomes and lessons

Remediation is described as of the linked source. A patch, account ban, or service restriction does not prove that every downstream system is safe. “Unknown” means the reviewed evidence did not establish a final outcome.

Lessons are my editorial interpretation of the reported failure, not a claim that a single control would certainly have prevented it. Sources, summaries, and conclusions can be revised when better evidence appears.

Follow or contribute

Daily source reviews check for new reports and outcome changes; the visible review date marks the latest published review. Use the RSS feed for entries added to the register, or export the filtered view as CSV. The JSON download keeps the source links and review metadata available for your own analysis.
